After Sept 1, 2003 a legible and complete set of fingerprints must be submitted with the Original Petition for Change of Name of Adult. Contact your local police department or the Texas Department of Public Safety to inquire if they have generic fingerprint cards and if they can take your fingerprints. After Aug 2006, you need to make a statement regarding sexual offender status regardless of who you are. This is covered in the update.
